New Patient Registration
If you wish to join the practice, the receptionist will be pleased to provide the appropriate forms for completion. The receptionist will check that you live in the practice area.
Patients living in the following post code areas are able to register with the practice: SO14 1, 2, 3 5 and 7. Patients outside of this area will be signposted to surgeries more local to their address.
Once your registration has been accepted, you will be offered a new patient health check, and you will be able to consult with any of the doctors in the practice by expressing your preference when you book an appointment.
You may not always be able to see the doctor of your choice immediately, but you will always be able to see another doctor or nurse within the practice when you need to.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice. Patients are asked to bring proof of identity and date of birth (such as photo id driving license or passport) when registering for the first time. This is used to check your details match with the information held on the NHS central patient registry and that your previous medical notes are passed on to the practice.
You will not be refused registration or appointments because you don’t have a proof of address or personal identification at hand. This also applies if you are an asylum seeker, refugee, a homeless patient or an overseas visitor, whether lawfully in the UK or not.
If you fall under one of the above mentioned patient groups, please download one of the ‘How to register with a GP’ patient leaflets below and bring it with you when you register with the practice. If you have problems registering with the practice, there is advice on what do to and where to go for further support.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is a printable version of this form, which you may fill out and bring in with your registration form.
When you register you will be asked for your mobile phone number and you will asked if you are happy for us to contact you by text.
With your consent we will text you to remind you of an appointment, to inform you of blood test results or to advise you if you are eligible for a flu vaccination during the Winter season.
It is very important to let us know if you change your mobile number.
